Practice/Insurance Guidelines

Even though there are a lot of strains on the relationship between providers and patients these days, the core of any professional relationship is trust and mutual consideration. What we promise you is honesty, transparency and compassion.

  • If you are a new patient please contact our office.
  • If you have an insurance plan that requires a referral it is your responsibility to get a referral to our practice from your primary care physician.
  • If you do not have a referral we will ask you to sign a waiver or pay for the office visit.
  • Attleboro Dermatology accepts a wide variety of insurance plans. Please make sure you understand the details of your insurance coverage. It is your responsibility to verify with your insurance carrier if we are "in-network" for your plan. We currently participate with: Aetna, Blue Cross and Blue Shield, CIGNA, Harvard Pilgrim Healthcare, Humana, Medicare, PHCS, Railroad Medicare, Senior Whole Health, Tricare, Tufts, Unicare, United Healthcare, and other commercial carriers.
    We currently are not participating with: BMC Healthnet Plan, Commonwealth Care, Health Safety Net, MA Health Connector Plans, Mass Health, Medicaid, Neighborhood Health Plan, Network Health
  • Co-payments are expected at the time of service.
  • If you ask for a service not covered by your insurance, we will quote you a price and payment is expected at the time of service.
  • Minors (17 years and younger) must be accompanied by an adult or guardian. In families that we know well, we have a consent to treat form which we ask guardians to sign.
  • Oral medications will not be refilled if you have missed your follow-up appointment.
  • Topical medications will be refilled at our discretion up to one year from your last visit.
  • Approved refills are completed within 2 business days and are not performed outside of regular business hours.
  • There is a charge for copying and mailing your medical records.

Laser
A pulsed dye laser which uses a specific wavelength of light to target benign vascular lesions of the skin such as rosacea, port wine stains and leg veins.

Photodynamic Therapy (PDT) for acne
A topical drug (Levulan) is applied to the skin and allowed to be absorbed. The drug is then activated by BLU-U® light.
